"DUCTAL" CARCINOMA IN SITU:

This lesion is the much discussed "DCIS". Note that it begins at the level of the lobule and not within the duct.

Slide 98. A 5 mm in diameter focus of ductal carcinoma in-situ is seen in this slice. The distal end of the terminal duct is distended by cells. The proximal terminal duct is labeled T.D. Below the terminal duct a cluster of coarse ductules is noted. A general rule is: Ductal hyperplasia is usually < 3mm in greatest diameter. DUCTAL CARCINOMA IN-SITU (DCIS) is >3 mm in diameter. Slide 100. The cell population is monotonous as if it is a single clonal derivative (cytologic criterion). The spaces are regular, of small size with rigid cell bars and protrusions (histologic criteria). This is ductal carcinoma in-situ (DCIS) CRIBRIFORM PATTERN. Photographed at 63x magnification
